[IMPORTANT: Sample Tracking-Holding Fee]
In order to serve you in a more efficient manner and streamline our sample receiving and log in, BBC Laboratories is adding a Sample Tracking/Holding fee for samples that are not submitted with an appropriately filled out Chain of Custody form.
Clients may submit a BBC Laboratories Chain of Custody form or their own company’s COC form. COC forms must include correct client information (name, address, telephone; fax or e-mail address is optional), all Client Sample ID’s, and analyses to be performed. Neglecting to provide any of the above mentioned information may result in a fee and a delay in the processing of your sample(s). Additionally, samples not packaged correctly and that require extra time to bag, re-bag, etc. may incur an additional fee. Fees may range between $10-$50, depending on the extra time spent filling out forms, tracking or calling clients, etc.
